OLDER ADULT SPECIALTY SERVICES (OASIS)

The OASIS program has been recognized by BCBS of MI as an "Outstanding Provider on Behalf of Michigan Seniors."

OASIS offers outreach and intervention for adults over sixty who are home-bound and isolated and at risk for depression, anxiety or substance abuse.  OASIS provides a comprehensive assessment in the privacy of the person’s home and jointly establishes a service plan that includes an activity program modified to their specific needs.

Through counseling and a re-introduction of activities that once brought pleasure into the life of the older adult, the program anticipates a renewed sense of purpose, an increased sense of joy and an enhanced quality of life.

The program incorporates two evidence-based models, Health IDEAS and Solution Focused Brief Therapy.  Staff are fully licensed and certified in older adult mental health services.
 

Services provided by OASIS include:

  • Comprehensive needs assessment
  • Six to ten visits in the home
  • Counseling
  • Behavior Activation Therapy
  • Psychiatric assessment if indicated
  • Case management
  • Community resource referrals
  • Educational trainings
  • Transportation to services and activities in the individual’s plan

Because the older adults that OASIS intends to serve are isolated and homebound, the program conducts outreach activities in collaboration with other service providers in order to identify those that may benefit from the program.  The program also accepts referrals from other sources.
